To submit an application, either for yourself or someone else, complete the form at the bottom of this page.
If you need help, you can call our office on 01634 383760 and we will complete the form with you over the phone. Alternatively, someone else can complete the form on your behalf such as a friend, family member, or support worker.
When we have received the form, your application will be reviewed and you will be contacted by our office within 24 hours (Monday - Friday only).
Per the information on our main Local Welfare Provision page, if you do not meet one of the three criteria on the form then you are not eligible and will be signposted elsewhere such as the Medway Household Support Fund.
Fraud Disclaimer
The Local Welfare Provision is a UK government scheme passed to local government level and administered by Citizens Advice Medway. As such, we must monitor all applications and awards for potential fraud and report any suspicions of fraud or confirmed fraudulent activities to Medway Council.
Both Medway Council and Citizens Advice Medway routinely monitor various outlets online and in the local community in an attempt to minimise abuse of this scheme. We are also passed potential fraud information by partner agencies and members of the public who are aware of this scheme.
Items awarded by this scheme cannot be sold for financial gain. By submitting this application you are agreeing to this condition.
Citizens Advice Medway does not accept liability for potential legal action that Medway Council may take should an applicant be found to have made a fraudulent application or be using the award for purposes other than those intended (eg: selling an item awarded by this scheme for financial gain).
Unfortunately, legal action has been taken against some applicants who have sold on items awarded under this scheme.
